Scope of the Job
The RN or LPN Clinical Nurse Program Coordinator for the Parkinson’s and Movement Disorders Center develops, facilitates, and/or coordinates patient care services, functioning as a patient advocate and resource, proactive case coordinator, and clinical educator for the patient. Works collaboratively with neurology and neurosurgery line leaders, medical staff, multidisciplinary, and others to design and implement and provide clinical support to targeted programs and initiatives. Provides consultation to staff and residents, participates in case conferences, and provides education to clinical team members as indicated. Works to increase program recognition for patients and community by organizing and participating in health fairs and outreach programs, and by reaching out to providers in the community to maintain awareness of program services and information.

Essential Functions

Review the appointment schedule to assure capacity for patients.
Work with Movement Disorders Physician(s) as necessary and perform any and all nursing duties within the scope of nursing practice, including management, care, and coordination of DBS, Duopa, and Botox Clinics and patient systems.
Work with Movement Disorder Specialist(s) to plan and implement care plans for patients, interfacing with other MDs, neuropsychologists, therapists, social worker, pharmacists, other medical staff, etc.
Educate families, caregivers, and community on appropriate care for patients and provide information on our program, services, and treatments in accordance with patient care plans.
Work with Program Attending to plan, develop, and conduct annual program simulations to facilitate education of residents. Manage and oversee all educational and outreach activities of the program.
Plan and implement activities to promote the program and services to providers in the community.
Work with Program Director to plan and conduct annual PD5K Run, annual Parkinson’s Symposium, Parkinson’s Interdisciplinary Clinic, and quarterly PD101 Classes.
Monitor Movement Disorders financial accounts for accuracy and coordinate payment for goods and services.
Oversee patient referrals to therapies and to outside resources.
Work with Program Director to develop and implement appropriate PD quality measures that will facilitate QI of processes within the Department. Generate quarterly report to present to Department QI Management Team.

SKILLS, QUALIFICATIONS, AND REQUIRED EXPERIENCE

Medical Terminology

A strong understanding of medical terminology is required to perform the day to day functions of this position.

Professionalism

Professionalism skills/adhering to rules and regulations, and maintaining valid professional certifications and protecting personal information.

Regulations/Policies

Knowledge of regulatory agency standards (e.g. Joint Commission, HRS), Nursing Standards and Florida Nurse Practice Act.

Knowledge of clinical practices and principles.

Experience

2 years experience in an acute care facility required

2 years Program promotion required
1 year working with community required
1 year Family counseling preferred
Up to 1 year EPIC or other Management system preferred

Education and Certifications
Current Florida LPN or Registered Nurse Licensure. Maintains current BLS certification. Completion of a standard medical terminology course would be helpful but not required. Valid driver’s license and insurance. Associates required. BSN preferred.
